Where to write for Vital Records

The National Vital Statistics System is responsible for the Nation's official vital statistics. These vital statistics are provided through state-operated registration systems. The registration of– births, deaths, marriages, divorces, and fetal deaths within the U.S. - is a State function.

Replacing a Consular Report of Birth

When a consular report of birth is lost, stolen, mutilated, or accidentally destroyed, a person may obtain a replacement from Department of State upon writing to:

Passport Services
Correspondence Branch (CA/PPT/PS/PC)
1425 K. Street, N.W.
Washington, D.C. 20524

Contents of written request:
Full name of the person;
Date and place of Birth;
Name(s) of the parent(s);
Location of the consular office that recorded the birth; and
Serial number of the consular report of birth, if known.
Affidavit:
The request must include an affidavit setting forth the circumstances surrounding the loss, theft, or mutilation of the previous document, and the appropriate fee (the same as for the originally issued report of birth, as determined by the current Schedule of Fees.


Replacing a Report of Death of an American Citizen Abroad

Additional copies of a Report of Death of an American Citizen Abroad can be obtained for a fee of ten dollars per copy. Please send a check or money order made payable to the Department of State, along with your written request, to the following address:
